Participation Agreement / Waivers

Participation Agreement: I give my permission for my child to participate in the 2018 Trip to Korea & Experience Korean School Program. I agree that my child will abide by all rules and regulations adopted and published by the Korean Education Centre and cooperated Korean schools relating to the operation and conduct of the program and the use of the facilities provided for the program. I understand that the failure of my child to observe these rules and regulations may result in his/her exclusion from participation in the program and a forfeit of all registration fees paid to the program.

Photo Consent: I give my consent for images of my child, captured during regular program activities through video, photo and digital camera, to be used solely for the purposes of the Korean Education Centre in promotional materials and publications, including the Korean Education Centre website & Social media, and I further agree to waive any rights of compensation or ownership thereto.

    

Homestay Consent: I give my consent for my child to participate in homestay during the program. I understand that all homestay activities will be conducted under the supervision of cooperated school manager and I have received all necessary information regarding my child's homestay family from the Korean Education Centre.

 

 

Transportation Consent: I give my consent for my child to participate in any off-site activity during the program. Transportation to and from these activities will be provided via school bus or public transportation which will be under supervision.

 

Youth Waiver Agreement: I give my permission for my child to participant in the 2020 Trip to Korea and Experience Korean School Program sponsored by the Korean Education Centre in Canada that he/she is currently registering for with the Seoul National University of Education and the Seoul Tourism High School in Korea. I understand that the failure of my child to observe these rules and regulations may result in his/her exclusion from participation in the program and a forfeit of all registration fees paid to the program. I represent that my child is physically able to participate in the program. I fully understand that his/her participation may entail the risk of physical injury. I agree to waive any claim of any kind of whatsoever, whether resulting from an injury or otherwise, and further agree to release, indemnify, and hold harmness the program. The Korean Education Centre in Canada, and its director, officer, employee, agents and/or representatives from any and all liability occuring as a result of his/her participation in the program. I will be personally responsible for any financial costs incurred as a result of his/her participation in the program, including, without limitation, medical expenses incurred as a result of any injury. Furthermore, I understand that the Korean Education Centre in Canada assumes no liability for lost, misplaced, stolen, and/or damaged property and I hereby agree to release the Korean Education Centre in Canada from such liability. The undersigned has read & voluntarily signed this waiver slip.
